You are acting as a physician on an internal medicine ward during your first on-call shift. A patient is transferred to your care after gastrointestinal bleeding, with a current hemoglobin level of 69 g/L. After evaluating all relevant factors, transfusion of 1 unit of red blood cells (RBCs) is indicated. You are aware that pre-transfusion preparation is a critical step in the entire process.

What will you focus on?

I will verify the documentation and the condition of the blood bag. I will ask the nurse to measure vital signs and perform the bed-side test.

I will verify the consistency between the patient’s documentation and the blood product, check the condition of the blood bag and the patient’s vital signs, and perform a bed-side test.

I will delegate verification of documentation consistency, inspection of the blood bag, and measurement of vital signs to the nurse. A bed-side test is not necessary, as compatibility is verified by the blood bank.
